Job Summary:
The physical therapist is a qualified therapist who directs, supervises, evaluates, and provides therapy services toward a rehabilitative plan of care to patients in the home health setting.
Responsibilities:
- Care Plan Development: Create and update individualized patient care plans to address physical therapy needs.
- Therapy Services: Evaluate patients by accurate diagnosis and prognosis, and treating patients to alleviate pain, improve function and enhance their ability to perform at maximum potential.
- Documentation and Patient Records: Create and maintain patient records and clinical notes in a timely manner.
- Collaboration and Development: Attend and participate in case conferences with local teams monthly, and actively participate in staff development opportunities.
- Supervision Duties: oversee Physical Therapy Assistants in accordance with Executive Council for Physical Therapy and Occupational Therapy Examiners.
- Communication: Communicate between agency, clients and your Area Support Manager to ensure well-coordinated approach to patient care.

Requirements
Qualifications:
- Bachelor's degree in a physical therapy related field
- Doctor of Physical Therapy degree from an American Physical Therapy Association (APTA) accredited program
- Active Texas licensure or certification as a Physical Therapist
- Preferred: one year of experience in a pediatric or home health physical therapy
